The inspector’s account

On 9/26/24, Licensing Program Analyst (LPA) M. Medina arrived at the facility unannounced to conduct a required annual visit. LPA explained the purpose of this visit and conducted a tour of the home with Administrator, Luis Raya.

Facility observed to be clean, odor free and a comfortable temperature. Residents were at day program at time of inspection. LPA observed resident bedrooms to have all required furnishings. All bedroom observed to have clean linen with additional linen available. Kitchen toured, food supply observed to be adequate for residents in care, all food observed to be properly stored and dated. Knives were stored and secured in hallway closet. Medications observed to have original labels and to be administered as prescribed. Bathroom toured, fixtures observed operational during facility tour. Water temperature measured at 105 degrees F.

Smoke detectors and carbon monoxide detectors observed to be operational during facility inspection. Fire extinguishers have a service date of 4/2/2024 and last fire drill conducted on 9/15/24 according to facility records.

Outside of facility toured. Facility has outside shaded and seating area for residents. All exits are free of obstruction and no hazards observed.

Staff files reviewed include current CPR/First Aid and CPI for all staff.

Administrator to submit an updated LIC500 to Fresno Regional Office no later than 10/11/24.

An exit interview was conducted with the Administrator. A copy of this report provided for facility records. .

No deficiencies observed during inspection visit.
