Facility Evaluation Report
On 11/25/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct a POC visit and Case management visit. LPA met with Licensee/ Administrator Susan Blanza. LPA toured the facility.
1. All medications locked in the repaired medication cabinet
Licensee/Administrator informed LPA Licensee is not the the assigned and appointed Administrator for the facility. Licensee informed and stated Petro Crisostomo is the assigned and appointed Administrator.
Current Administrator certificate for Administrator and updated Lic 500 is to be submitted to Fresno CCL by 11/26/25. An exit interview was conducted. A copy of this report was provided to Licensee who confirms receipt of this report.
