Facility Evaluation Report

Licensing Program Analyst Shawna Doucette arrived at the facility unannounced for a complaint investigation. During the course of the investigation, it was found the facility does not have a current Administrator that meets the qualifications. LPA met with Staff Griselda "Gracie" Ramirez.

LPA reviewed files submitted for Staff Griselda "Gracie" Ramirez to be designated as Administrator. Facility needs to submit a letter to Licensing to show how staff meets the qualification of providing residential care to the elderly. Staff does have a current Administrators certificate.

An exit interview was conducted with Staff Griselda "Gracie" Ramirez and a copy of this report with plan of correction and appeal rights were provided.
