Facility Evaluation Report
On 06/18/2025, Licensing Program Analysts (LPA) J. Duarte conducted an unannounced
annual required inspection visit. LPA introduced self, and stated purpose of visit, and was allowed entrance by staff Wafik Gerges. LPA and Wafik contacted Licensee, Dinafay Crandell via telephone and she arrived shortly after. There are currently four residents residing in the facility, three residents were present during the visit. Staff stated that one resident was at day program.
LPA observed an infestation of tiny insects on the living room couch. Administrator was notified and stated that the facility has pests control services and will provide documentation. In addition, LPA asked the administrator to have pest control identify what type of insects/pests they are and treat for that specific insect/pest.
Due to time restraints, LPA will return at another time to complete annual inspection visit.
The report will be provided to the Administrator via email and a hard copy will be mailed to the facility.
