Facility Evaluation Report

On 8/10/22 at 9:00 AM, Licensing Program Analyst (LPA) Malia Thao arrived announced to conduct a follow-up prelicensing inspection. LPA met with Licensees Jason Johnson and Jaimy Johnson and was granted entry.

LPA received a Plan of Action to address the shared backyard space, as an exterior fence would not be installed to separate the facility's shared backyard space with the adjoining building units. Hot water in hallway bathroom measured at 114.6 degrees F.

All pre-licensing requirements have been met. LPA will notify CAB in Sacramento for final review prior to license being issued.

Exit interview was conducted. A copy of this report was given to Licensee Jaimy Johnson, whose signature confirms receipt of this report.
