Licensee Jason Johnson and Supervisor Diana Diaz

Staff did not prevent resident from eloping from facility

On 09/25/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to deliver finding on the above allegation. LPA was greeted by staff Melanie Zavala and granted entry. LPA introduce self and stated the purpose of the visit. Licensee Jason Johnson and Supervisor Diana Diaz arrived shortly.

During the course of the investigation, interviews were conducted, records were reviewed, and facility was toured. It was confirmed that C3 and C2 had AWOL from the facility on separate occasions.

Based on interviews which were conducted and records reviewed, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Exit interview conducted. A copy of this report and appeal rights was provided to Licensee.
