On 8/22/23 at 1:27 PM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to conduct a case management - other inspection to return a resident file. LPA explained reason for inspection and met with Facility Manager Nicole Morehead. Administrator Jonathan Johnson was not available for the inspection.
Exit interview conducted. A copy of this report was left with Facility Manager, whose signature confirms receipt of this report.
