02/03/2026 and conducted by Evaluator Katie Brown

COMPLAINT CONTROL NUMBER: 24-AS-20260203162450

PATHWAY ASSISTED LIVING/ WESTCHESTER GARDENS

Licensing Program Analyst (LPA) Katie Brown arrived unannounced to conduct a subsequent visit and deliver investigation findings. LPA explained the reason for the visit and discussed the allegation with Administrator (AD) Realiza Angales.

During the visit, LPA observed Resident (R1) and conducted interviews. Interviews reveal that on 2/2/26, R1's Physician advised hospital evaluation due to the worsening of a serious infection requiring isolation and different medication. On 2/3/26 the hospital confirmed the infection diagnosis and informed that R1 was medically cleared to return home. The facility could not provide the level of care or isolation R1 required. R1 was admitted to a Skilled level facility until cleared of the infection.R1 returned to the facility on 3/3/26. This Agency has investigated the allegation listed above. We have found that the allegation is UNFOUNDED, therefore we have dismissed the allegation.

An exit interview was conducted and a copy of this report was provided.
