The inspector’s account

On 07/03/25, Licensing Program Analyst (LPA) J. Duarte and M. Medina arrived unannounced to conduct an annual inspection. LPA explained the reason for the inspection and met with Program Coordinator (PC) Joy Bowers. She advised that clients and staff have a planned Fourth of July Holiday BBQ and will be going to the water park to celebrate and have a lunch.

LPAs toured the facility with PC. The day program was maintained at a temperature of 76 degrees F. Clients were present at the day program during this inspection and they were observed watching television, putting puzzles together and engaging with staff. PC stated that there are 35 clients in the facility and it is a three to one ratio. LPAs observed adequate seating and lighting throughout the facility. No passageway obstructions or fire hazards were observed inside or outside of the facility. Client restrooms were toured and were observed to be clean, and operational. LPAs toured the Assisted Technology Services (ATS) room, game room, sensory room that includes activities such as a sensory wall, puzzles, and a sensory lighted fish tank, copy room, staff kitchen, and individuals kitchen area were toured.

The staff room observed to have a key pad lock which stores chemicals and other program supplies. The staff room in inaccessible to individuals. The staff kitchen is locked and secured, and stores chemicals under sink as well as sharps utilized for cooking. The facility has first aid kit backpacks throughout the facility. Fire extinguishers present throughout the facility with service dates of 08/15/24. The smoke and carbon monoxide detectors were tested and observed to be operational during inspection. The last fire drill was conducted on 6/27/25 according to facility records. The facility does not administer medications to clients.

Due to day program festivities, LPAs will return to complete inspection tool and review a sample of staff and client records.

No deficiencies were observed during this inspection.

An exit interview was conducted with PC and a copy of this report provided for facility records.
