Facility Evaluation Report

On 04/14/25, Licensing Program Analysts (LPAs) M. Yang and J. Duarte arrived unannounced to conduct a health and safety check. LPA met with Administrator Joye Mackey and announced the purpose of the visit. The purpose of the visit is to address an incident that was reported to the department.

On 04/05/25, the department received written incident report from the facility reporting on 04/04/25, C1 had called police and alleged S1 hit C1. The department conducted interviews with C1 and Administrator. C1 confirmed staff did not hit client, the client was not happy with Regional Center and S1 at the time leading the client to call the police and made a false accusation of S1 hitting the client.

The information provide will be reviewed; a follow up case management will be conducted if necessary.

An exit interview was conduct. A copy of this report was provided to Administrator, whose signature on this
