Facility Evaluation Report

On 03/14/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduce self, stated the purpose of the visit and met with staff Cassandra Reveles. Administrator Joye Mackey was called. Residents were observed in the common area and in the bedrooms. LPA toured facility with staff. Two clients were present during inspection. Administrator arrived later during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. Fire extinguisher was observed with a service date: 01/18/24. Last fire drill completed on 02/14/25. An adequate supply of perishable and non-perishable food was observed. Perishable expired food was observed. Refrigerator temperature is maintained at 30 degrees F and freezer temperature maintained at 10 degrees F . All clients’ bedroom were toured and observed to have the required furnishings and with adequate lightening. All bathrooms were toured. Bathrooms observed operational and functional. Hot water temperature was tested at 90.4 degree F in the bathroom 1, 82.8 degree F in bathroom 2, and 82.5 degree F in master bathroom. Extra linens were observed. Chemicals observed stored and locked in garage cabinet.

Outside of facility toured and observed to be free of debris. Side exit observed self-closing and self-latching. Adequate outdoor seatings available for clients. Medications were observed locked in kitchen shelf. Medications check and MARs reviewed. Sample of clients’ and staff files were reviewed.

A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6.

Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 03/20/25: Lic 308, Lic 309, Lic 500, Lic 9020, Lic 610D, control of property, and current Administrator certificate. A copy of this report and appeal rights was provided to Administrator, whose signature on this form confirms receipt of these report.
