Facility Evaluation Report
On April 22, 2026 Licensing Program Analysts (LPA) B. Miranda conducted an unannounced visit today for the facility’s annual inspection. LPA introduced themselves and was allowed entrance into the facility. LPA met with Joye Mackey- Administrator.
Facility is licensed for 6 residents and has a current census of 3. Water temperature was checked in the kitchen which read at 120 degrees Fahrenheit and the common bathroom which read at 119.8 degrees Fahrenheit. Fire Extinguisher was serviced January 5, 2026 and is within the safety regulation period. Smoke and carbon monoxide detectors were tested and in working order.
Joye Mackey’s Administrator Certification expires July 11, 2026. Staff files were reviewed, complete, and current. Resident files were reviewed, complete, and current. Facility provided a log for disaster drills that are conducted monthly. First aid kit on site and complete. LPA observed one cleaning product & scissors in the staff desk to be in unlocked drawer. LPA also observed tools (screw drivers) and hammer to be accessible to residents. There is a locked cabinet for medications.
LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms, bathrooms, medication cabinet, kitchen, garage and outdoor areas. Bedrooms were properly furnished with adequate lighting. LPA observed the master bathroom to smell of urine. LPA observed couches to be stained or have spillage. Food supply is adequate for 2-days perishable and 7-days nonperishable.
