Facility Evaluation Report

On March 4, 2026 Licensing Program Analyst (LPA) B. Miranda & D. Boyd arrived to the facility unannounced to follow up on an incident report previously submitted to the Dept. LPAs met with Supervisor Dodie Franklin and Joye Makey.

While interviewing it was stated R1 threatened staff during the incident. R1 was arrested and later released. Facility was not notified when R1 was released. R1 has since returned to the facility. Dodie explained R1 was placed in the facility as an emergency placement. Dodie stated there was not an appraisal completed for R1 before being accepted into the facility.

Citation was issued today due to appraisal not being completed before accepting R1 into the facility.

LPA will return at a later date to issue additional citations if warranted.

Exit interview was conducted and a copy of this report LIC809, LIC809D, and appeal rights were provided to Supervisor Joye Makey .
