Facility Evaluation Report

On 02/25/2026 Licensing Program Analyst (LPA), M Vega to conduct an unannounced case management visit. LPA was granted entry into facility by Staff 1 (S1). S1 notified Administrator (AD) - Joye Mackey and Administrator (AD) - Dodie Franklin of LPA arrival and Administrators arrived a short time later.

This case management visit is being conducted, need clarification regarding Client 1 (C1), LPA will request the following: (R1: 602, IPP and Hospital documents). Facility tour conducted, no health and safety issues observed at the time of this visit.

At this time, case management does not need further review. Administrator (AD) - Joye Mackey and Administrator (AD) - Dodie Franklin was informed that LPA may need additional documentation and no deficiencies cited during todays visit.

Exit interview completed with Administrator. A copy of this report was provided for facility records.
