The inspector’s account

On June 2, 2026 Licensing Program Analysts (LPAs) B. Miranda & A. Porter arrived at the facility unannounced to conduct a case management visit regarding R1.

LPAs spoke with Merly Alejo RN. On March 9, 2026 R1 was sent to the hospital due to low saturation. Facility's Respiratory Therapist attempted to increase saturation level, but was unsuccessful. Respiratory Therapist contacted R1's Nurse Practitioner and was told to send R1 to the hospital.

Merly stated R1 is prone to pneumonia due to being bed bound. Staff repositions R1 every 2 hours. Facility has a current care plan for R1 which is reviewed/updated every three months. After eating staff have R1 in their wheelchair for an hour.

No deficiencies cited during today's visit.

Exit interview was conducted and a copy of this report LIC809 was provided to Merly Alejo Facility RN.
