Facility Evaluation Report
On 2/18/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA arrived, introduced self, stated purpose of visit and allowed entrance by House Manager. LPA contacted Administrator, Patrick Nankil who arrived a short time later to meet with LPA.
Currently, there are four (4) residents in care. Two residents were at day program at time of inspection. Residents attend day program Monday through Friday 8:00 AM to 3:30 PM. Two (2) residents were observed to be present and relaxing in their bedrooms. Facility observed to be clean, well lit and odor free. Adequate seating and lighting observed in the living room, family room and dining room. Resident bedrooms toured, bedrooms observed to have all required accommodations. Resident bathrooms toured, LPA observed shower area to have grab bars, shower chair and skid resistant mat available. Fixtures observed operational, water temperature at 110 degrees F. Kitchen toured, LPA observed a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Medications observed to be kept in a locked cabinet in dining room. Resident medications reviewed, medications observed to have original labels and appear to be administered as prescribed.
All cleaning supplies are locked and secured in garage. Smoke detectors and carbon monoxide observed to be operational during today's inspection. Fire extinguisher present with a purchase date of 10/02/2025.
Outside of facility toured. Pool is surrounded by gate that is locked, secured, and inaccessible to residents. All exits open free of obstruction, no hazards observed.
No deficiencies observed during today's visit. Exit interview conducted and a copy of report provided for facility records.
