Facility Evaluation Report

On 5/28/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management. LPA met with Administrator, Patrick Nankil to conduct visit.

LPA conducted visit to follow up on 2 self reported incidents for R1 that occurred on 1/15/2025 and 2/03/2025, resulting in falls with R1. LPA reviewed centrally stored medication records and R1's file during case management visit.

In December 2024, R1 was placed on medication at 1mg, in January 2025 it increased to 2mg, then increased again to 3mg by end of January 2025. After subsequent fall on 2/03/2025, R1 was re-evaluated by psychiatrist and medication was reduced back to 2mg. R1 is currently stable and has not had any additional incidents.
