Facility Evaluation Report
On 07/31/2026, Licensing Program Analyst (LPA) J. Duarte arrived at the facility unannounced to conduct the required annual inspection. LPA was greeted by Program Director, Maria Montes. LPA stated the purpose of the visit, and was allowed entry into the facility.
LPA toured the facility with program director. The facility was observed to be free from odor. Facility temperature measured at 73 degrees F. Disinfectants and cleaning supplies were observed to be locked in a room and inaccessible to clients. The hot water in the restrooms was tested and measured 105 degrees F.
LPA toured eight client rooms were 51 clients were observed having snacks, drawing, making puzzles, and engaging with each other. LPA observed 15 staff and one volunteer with clients. Scissors were observed locked in a drawer in room one. The facility has a changing room, staff break room, locked medication room, snack room, laundry room, and a kitchen. The facility has a an outdoor patio with seating available for clients.
Emergency disaster plan and procedures are in place. Last drill was earthquake and was conducted on 07/27/2026. Doors and passageways were observed to be free from obstruction throughout the facility. Fire extinguishers were observed with a service date 03/17/2026. First Aid kits were observed to have the required items. A sample of staff and client files were observed to have the required training and forms. Medication along with the MARs were reviewed and reflect medication is being administered as prescribed.
