The inspector’s account

On 08/08/24, Licensing Program Analysts (LPA) L. Salazar arrived to the facility unannounced to conduct the required annual inspection. LPA was greeted by the receptionist, introduced self and stated the purpose of the visit. LPA was allowed entry and met with Program Director (PD) . Required postings were observed in the lobby.

LPA toured the inside and outside of the facility with PD. No fire hazards or passageway obstructions were observed. There are no bodies of water on the premises. Fire extinguishers were observed with a service date of 03/20/2024. Emergency Disaster plan was observed to be updated and complete. Emergency Disaster Drill logs were observed for all staff. Last drill was an earthquake drill conducted 07/31/24. Facility temperature was 74 degrees F.

LPA observed 6 large classrooms (identified by colors) and 6 bathrooms with multiple stalls in each, accessible to clients in care. LPA observed approximately 24 clients per room with 3 to 4 staff present. Classrooms were observed to have sufficient seating and adequate lighting. LPA observed restrooms to be clean, have operational lights, running water and are free from odor. Hand washing signs were observed in each bathroom. Hot water measured at 106.8 degrees F.

LPA observed a large common area known as the "Rec Room", where clients participate in group activities and have lunch. No lunch is prepared on the premise. Facility has a food program through USDA that provides breakfast, snacks and lunch. There is also a snack bar/store available for clients to purchase their own food.

Medications are stored on site in a locked medication room. LPA reviewed a sample of client files and personnel records. Facility files were observed to have the required forms and training documentation. Cleaning supplies were observed to be locked in the janitor's closet and locked cabinets in the laundry room. First aid kit was observed and contained all required items.

The following documents were received at the time of visit: LIC 500, Emergency Disaster Plan (LIC 610D) and LIC 9020 (Client Roster). An exit interview was conducted with Administrator. A copy of this report was discussed and provided at the time of visit. No deficiencies cited on today's visit.
