Facility Evaluation Report
On 06/22/2026, Licensing Program Analysts (LPA) J. Duarte conducted an unannounced Annual Required Inspection. LPA arrived and there was no answer at the door. LPA contacted Licensee Helen Houck by telephone, and she said staff would arrive shortly. Administrator Claudia Yvonne Ramirez arrived shortly after. LPA advised the purpose of today's visit and toured the facility with the administrator.
The facility currently has four residents in care; however, they were not at the facility at start of inspection. The administrator stated that the residents were at day program. Residents attend day program Monday through Friday.
LPA toured the facility with Claudia. The facility was observed to be clean, odor free, and the temperature was at 77 degrees F. Resident bedrooms toured; resident rooms observed to have required furniture. Resident bathrooms toured, fixtures observed to be operational and showers and toilets had accessible grab bars. Water temperature measured at 109 degrees F. Kitchen toured, facility observed to have a seven day supply of non-perishable food and a two day supply of perishable food. The sharps were observed to be stored in a key locked drawer. Medications were also observed to be locked and secured in a key locked kitchen cabinet. Living room and dining room observed to have adequate seating for all residents. All chemicals and detergents observed to be secured in a locked in hallway closet and under the sink in the hallway bathroom. Extra linen was observed to be stored in a hallway closet.
Outside of the facility was toured, exit opens and is free of any obstruction with a self latching gate. There is a shaded seating area in the back patio.
