Facility Evaluation Report
On 6/04/2026, Licensing Program Analysts (LPAs) M. Medina and J. Duarte conducted an unannounced Annual Inspection. LPAs stated the purpose of the visit and were allowed entry into the facility. LPAs met with Administrator and House Manager to conduct facility visit.
LPA Medina reviewed a sample of staff and resident files. LPA reviewed Emergency Disaster plan and observed the binder to have the required updated information.
LPA Duarte will document the physical plant tour, inspection tool results, and any deficiencies if observed on a separate report.
An exit interview was conducted with Administrator. A copy of this provided via e-mail.
