Facility Evaluation Report
On 06/04/2025, Licensing Program Analysts (LPAs) J. Duarte and M. Medina conducted an unannounced
annual required inspection visit. There was no answer at the door. LPAs contacted Administrator Adam Tauchen, whom stated that there are two residents; however, they are at day programs and staff are not at the facility. Adam stated that he would contact Administrator Gerald De Claro to be available for this annual inspection. Administrator Gerald arrived shortly after.
There are currently two residents residing in the facility. Both residents were at day program at start of inspection. Facility toured with Administrator and observed to be well lit, odor free, and comfortable temperature set at 70 degrees F.
Facility living room and dining room observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. LPA observed R1's and R2's bedroom sliding glass door that exits towards the backyard to be secured closed with a wood stick in the track. Resident bathrooms toured, water temperature measured at 110 degrees F. Showers have shower chairs and skid resistant mats available.
LPAs observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. Facility observed to have a two-day supply of perishable food and a seven-day supply of non-perishable food. Medications are centrally stored and locked in a kitchen cabinet. All medications observed to have original labels. Medication and MARs were reviewed and appear to be administered as prescribed.
