The inspector’s account

Licensing Program Analysts (LPA) Jimmy Duarte arrived at the facility unannounced to conduct an Annual Inspection. Staff were not present at the time LPA arrived. LPA contacted AD via telephone. AD arrived shortly after and LPA was granted entry. All residents were at day program.

LPA conducted a tour inside and outside of facility. Facility observed to be clean, and at a

temperature of 73 degrees F. Common areas were furnished well with adequate seating and lighting available. Kitchen toured, and was clean and safe for food preparation. Food supply checked, LPA observed an adequate supply of perishable and nonperishable food.Resident rooms were toured and were observed to have required furnishings. LPA observed an adequate supply of linen. Hot water measured at 107.7 degrees F in the hallway restroom and at 106.3 degrees F in the restroom to bedroom #1.

Exterior tour conducted, all exits open and free of obstructions. Side gate was

observed to be self-latching. Backyard patio has seating available for residents. Pool is fenced and gated with lock.

Fire extinguisher serviced on 10/10/24. Smoke detectors and carbon monoxide detectors observed

operational during today’s inspection. Facility has a pull station fire alarm. Facility has sprinkler system which was last inspected 12/05/24. Last fire drill conducted on 08/20/25. All cleaning supplies are locked in a cabinet in the laundry room. Knives are locked in a kitchen drawer.

Medication and MARS were reviewed, which reflect medication is being administered as prescribed. All resident files were reviewed and a sample of staff files were reviewed.
