The inspector’s account

On 08/06/2026, Licensing Program Analyst (LPA) J. Duarte arrived unannounced at the facility to conduct an Annual Inspection. There was no answer at the door. LPA contacted Administrator (AD) Adam Tauchen via telephone and he arrived shortly after with Direct Care Staff Josephine Insauriga, AD Joseph Washington, and House Manager Ricardo Vicente.

LPA conducted a tour inside and outside of the facility with AD Adam Tauchen and AD Joseph Washington.. The facility was observed to be clean, odor free, and at a temperature of 76 degrees F. The common areas were furnished well, with adequate seating, and lighting available. The Kitchen was clean and safe for food preparation. The facility had a two-day supply of perishable and a seven-day supply of non-perishable food. Sharps were observed to be locked in a cabinet next to the stove. Medication was also observed to be centrally stored in a lock cabinet in the kitchen. Medication and MARS were reviewed for two residents and logs were complete.

The resident rooms were observed to have the required furnishings. LPA observed an adequate supply of linen. The hot water temperature was tested and observed at 143 degrees F in the main facility restroom and at 83 degrees F in R1 and R2's restroom.

LPA toured the exterior of the facility, the side gate was observed to be self-latching. The rolling gate that leads towards the house driveway slides open and slides to closes.
