Facility Evaluation Report

icensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct the Required Annual Inspection. LPA disclosed the purpose of the inspection and was granted entry into the facility by Program Manager Calvin Shephard. LPA met with Program Manager Calvin Shephard and Gerald De Claro.

A tour of the facility was conducted. The facility was set at 71 F temperature and free of passageway obstructions inside and outside. The water temperature was measured at 110.8 F.

Kitchen was toured. Cleaning supplies were in a locked storage room. Smoke detectors and carbon monoxide detectors were checked and operating. Facility has pull station fire alarm. Fire extinguishers were charged and had service dates of 09/24/25. Fire drill was last completed on 6/5/25.

There was outdoor seating for the clients. Facility has numerous activity rooms, where clients were observed to be engaging in activities.

Client and staff records were reviewed. Medications were reviewed. Current first aid and CPR were on file for staff.

An exit interview was conducted with the Program Director and a copy of this report was provided.
