The inspector’s account

On 10/29/2025, Licensing Program Analyst (LPA) J. Duarte conducted an unannounced

annual required inspection visit. There was no answer at the door. LPA contacted Licensee Helen Houck via telephone and she stated that she will ask House Manager (HM), Claudia Yvonne Ramirez to report to the facility. HM and licensee arrived shortly after. There are three resident that reside at the facility; however, they were all at day program.

LPA toured the facility with HM and observed the facility to be well lit, odor free, and at a temperature set at 73 degrees F. Facility living room and dining room were observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. The hallway restroom hot water measured at 105.3 degrees F. The hot water to the restroom attached to the bedroom measured at 109 degrees F. Showers have a shower chair and skid resistant mats available. The facility has a supply of extra linen and comforters in the hallway.

LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. Facility observed to have and adequate supply of food for the residents in care. Medications are centrally stored and locked in a kitchen cabinet. All medications observed to have original labels and be administered as prescribed. The facility has a freezer in the garage with an additional supply of food. Chemicals are locked in a cabinet in the garage.

Outside of facility toured. All exits open free of obstruction. No hazards observed. Facility has seating and a shaded area for residents. The facility has a pool and jacuzzi in the backyard that are gated and locked.
