Facility Evaluation Report

On 08/19/25, Licensing Program Analyst (LPA) M. Yang conducted case management visit to the facility. LPA introduce self, stated the purpose of the visit, and met with Executive Director Mireya Melchor.

The purpose of the visit is to conduct a health and safety visit on the residents in care and follow up on written incident report regarding an incident that occurred on 08/10/25, an physical altercation between R1 and R2. The department was notified of a concern on staff providing care for residents. Copies of records were received, interviews were conducted and the facility was toured.

The information provided will be reviewed; a follow up case management will be conducted if necessary.

An exit interview was conducted. A copy of this report was provided to Executive Director, whose signature confirms receipt of this report.
