Facility Evaluation Report
On 05/20/2026, Licensing Program Analyst (LPA) M Vega conducted case management- deficiency visit to the facility. LPA introduced self to Staff 1 and was granted entry into the facility, met with Facility Manager - Cami Watkins - and stated the purpose of the visit.
The purpose of the visit is to address incident that was reported to the department on 05/12/2026 where Client 1 (C1) AWOL from the facility on 05/10/2026. The facility was aware that C1 went AWOL however was not able to follow Client. The facility contacted Kern County emergency services, and the Sheriff was able to locate and persuade C1 to return to the facility.
Therefore, as mentioned, C1 went AWOL from facility. As a result, a deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached LIC 809D. An immediate civil penalty of $500.00 was issued, see LIC 421IM.
An exit interview was conducted. A copy of this report and appeal rights was provided to Facility Manager, whose signature confirms receipt of this report.
