On February 25, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to follow-up on an incident that was previously reported to the Dept.
LPA interviewed staff and residents at the facility regarding previous Administrator. LPA will follow-up with LPM to see if a follow-up visit is warranted.
Exit interview was conducted and a copy of this report was provide to Vivianna.
