Facility Evaluation Report

On 04/26/25, Licensing Program Analysts (LPAs), L. Salazar and M. Garza arrived to the facility unannounced to conduct the required annual visit. LPAs were greeted by House Manager Carmen Martinez, stated the purpose of the visit and were allowed entry into the facility. Administrator was available via telephone. During the visit, LPA Salazar reviewed incident reports.

On 04/05/25, It was reported that Resident R1 complained of tooth pain on 04/05/25 and was taken to Urgent Care due to swelling in their face. Dr. administered a shot for pain and also prescribed antibiotics. A treatment plan is pending and a request for approval for anesthesia surgery was sent on 04/14/25 for the scheduled appointment on 06/06/25.

An exit interview was conducted with House Manager and Administrator. A copy of this report was sent via email on 04/26/25. No deficiencies cited on today's visit.
