Facility Evaluation Report
On 03/26/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct case
management visit for the purpose of checking on the health and safety of residents in care. LPA introduced self, stated the purpose of the visit, and requested to meet with administrator. LPA met with Licensee/ Administrator Socorro Ann Telmo.
LPA conduct tour with Licensee. A knife was observed in the kitchen sink unlock. Chemicals were observed unlocked under kitchen sink and in the laundry room. Cleaning solutions and chemicals were observed in the garage cabinet unlocked.
A deficiency is being cited, per California Code of Regulations, Title 22, Division 6, see attached Lic 809D. A civil penalty is being assessed see attached Lic 421IM.
Exit Interview conducted. POC was discussed with Licensee during visit. A copy of this report and appeal rights was provided to Licensee, whose signature on this form confirms receipt of this report.
