The inspector’s account

On 6/09/25, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA Medina was allowed entrance by Direct Care Staff. Licensee/Administrator Liza Pinono contacted by telephone and arrived a short time later to conduct visit.

Facility currently has five (5) residents in care. All residents were present at time of inspection. Facility observed to be a comfortable temperature and have adequate lighting throughout. Facility tour began in resident bedrooms, all bedrooms observed to have required accommodations. Resident bathrooms observed to be clean, and fixtures observed operational. LPA observed shower/tub to have shower chairs, grab bars and slip-resistant mats available. LPA observed water temperature at 108 degrees F at time of inspection. Dining room and living room have adequate seating available for all residents in care. Kitchen toured, LPA observed facility to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available for residents. Food observed to be properly stored and labeled. Medications observed to be locked and secured. Medication observed to have original labels and to be administered as prescribed. Smoke detectors and carbon monoxide detectors observed to be operational at time of visit. Fire extinguisher present with a service date of 12/29/25. All cleaning supplies observed to be locked and secured in laundry room. Staff and resident files reviewed. All facility staff who require caregiver background checks have received criminal record index clearance or exemptions.

Outside area toured. All exits open free of obstruction. There is seating and large shade umbrella available for residents. Fire exit observed to be self latching and unobstructed at time of inspection.

No deficiencies cited during annual inspection.

Exit interview conducted with Licensee/Administrator and a copy of report provided for facility records.
