Facility Evaluation Report
On 10/31/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an Annual
Inspection. LPA introduced self, stated the purpose of the visit, and was greet by staff Haley Santner and staff Janete Agpalza. LPA was granted entry. Administrator Rosalinda Cua and Licensee Ronald Duran was called and arrived shortly. Three clients were present upon LPA arrival. One client arrived later during inspection.
LPA toured facility with Licensee and Administrator. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside and outside. Medications observed kept locked in kitchen shelf. Sharps were locked in kitchen drawers. Clients’ MARS were reviewed, and medications were checked. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 37 degrees F. Fire extinguisher was observed with a service date of: 10/23/24. Fire drill last completed on 10/28/24. Cleaning chemicals was observed locked in garage cabinet and under kitchen sink. Extra linens observed in hall closet. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed functional. Grabbed bars and non-skid mat observed in bathroom. Hot water temperature was tested range 105.5 and 106.3 degrees F. in bathroom 1 and range at 106.4 and 105.7 degrees F. in master bathroom. First aid kit observed with all required items.
Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. All clients’ and staff file reviewed to have all the required documents. Carbon monoxide and smoke detectors were tested and observed to be operational.
No deficiency observed during inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 10/06/24: Lic 308, Lic 400, Lic 402, Lic 500, Lic 9020, Lic 610D, and current Administrator certificate. A copy of this report was provided to Administrator, whose signature on this form confirms receipt of these report.
