Facility Evaluation Report

Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with facility Program Director, Brandy Bailey , and explained the purpose of today's visit.

The facility submitted an Incident report documenting on April 5th 5:13pm, Staff 1 Reported incident to Bakersfield Police Department, alleging Resident 1 (Individual) exited their room and began walking toward the kitchen. Staff 2 pushed Resident 1 back toward their room, and Resident 1 fell to floor. Staff 2 assisted Resident 1 to stand, escorted back to their room, and closed the door. Staff 2 was placed on unpaid leave.

The following deficiencies are being cited Per Ttile 22 Regulations, and exit interview conducted with facility , Program Director, Brandy Bailey, and a copy of this report provided.
