COMPLAINT CONTROL NUMBER: 24-AS-20240216173627

Client sustained an unexplained injury due to lack of care from staff

On 05/10/24, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to deliver findings on the above allegation. LPA was greeted by Staff S1, stated the purpose of the visit and was allowed entry into the facility. Program Director (PD) and Area Director arrived to the facility during the visit.

During the investigation, LPA conducted interviews and records review. Based on the information received, although the allegation may have happened, there is not a preponderance of evidence to prove that the alleged violation occurred, therefore the allegation is Unsubstantiated. Exit interview conducted. No deficiencies cited.
