COMPLAINT CONTROL NUMBER: 24-AS-20250305120608

Licensing Program Analyst (LPA) Shawna Doucette conducted an unannounced complaint visit and was granted entry by Administrator John Noriega. LPA explained the purpose of the visit. LPA delivered findings.

LPA reviewed surveillance video and conducted interviews.

Based on interviews and surveillance video it is unknown how C1 obtained the unexplained bruising. Surveillance video shows C1 having behaviors that may or may not have caused the bruising. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
