08/27/2024 and conducted by Evaluator Alexandria Walton
COMPLAINT CONTROL NUMBER: 24-AS-20240827101249
On 10/01/2024, Licensing Program Analyst (LPA) Walton arrived unannounced to deliver findings on the above allegation. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator, John Noriega.
During the course of the investigation, LPA conducted interviews and reviewed records. Interviews revealed that S1 escorted C1 to the restroom to assist with activities of daily living and observed a bruise on C1’s left leg near the calf. S1 conducted a brief body check and escorted C1 back to the group.
Based on interviews conducted the allegation: Violation of personal rights is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
No deficiencies issued during today’s inspection. Exit interview conducted. A copy of this report was discussed and provided to Administrator, John Noriega, whose signature on this form confirms receipt of this document.
