Facility Evaluation Report

On July 13, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit regarding an incident that was previously reported by the facility. LPA introduced themselves and was allowed entrance into the facility. LPA met with Chief Operations Officer Ana Leheny .

The facility reported to the Dept that on July 3, 2026 there was an incident that took place regarding staff and C1's wallet. LPA was able to review the video while at the facility. The video showed C1 looking inside their fanny pack and their wallet falls out and drops to the floor. Staff then informs C1 of their wallet being on the floor, C1 picks up their wallet and places the wallet back in the fanny pack.

LPA requested a copy of C1’s IPP, admission agreement, physician’s report, and the facility’s plan of operation/program design.

At this time no citations were issued. A follow-up visit may be warranted at a later date once all documents have been reviewed.

Exit interview was conducted and a copy of this report LIC809 was provided to Ana Leheny.
