On 5/31/2024 Licensing Program Analyst B. Miranda arrived at the facility unannounced to conduct a case management visit.
There was no answer at the facility. LPA attempted to call Administrator Rhonda Glenn. The call was unsuccessful and LPA was not able to leave a voicemail or message.
LPA left the facility without being able to contact staff or administrator. LPA will follow-up.
