Facility Evaluation Report
On 10/182025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required inspection. LPA identified self, stated purpose of visit, and allowed entrance by Licensee/Administrator Joel Jimenez to conduct today's inspection.
Facility observed to be clean, odor free, and a comfortable temperature. LPA observed common areas of the facility to have adequate seating and lighting available. Residents observed to be watching television in the living room and some relaxing in their bedrooms. Resident bedrooms toured, all bedrooms observed to have required furnishings available. Resident bathrooms toured, all fixtures observed operational, shower observed to have skid resistant mats, shower chair and grab bars available. Kitchen toured, facility observed to a have a 2-day supply of perishable and a 7-day supply of non-perishable food available. All food items stored in refrigerator observed to be properly stored and dated. All knives observed to be locked and secured. Medication observed to be locked and secured in hall closets. Medications observed to have original labels and to be administered as prescribed.
Facility is equipped with a pull station, fire extinguisher has a service date of 12/26/2024. Smoke detectors and carbon monoxide detectors tested and observed operational at time of inspection.
Outside area toured, facility has covered patio with seating for residents. Shed in the backyard observed to be locked and secured. No hazards observed.
LPA reviewed resident and staff files during inspection.
An exit interview was conducted with Administrator and a copy of this report was provided during visit.
