Facility Evaluation Report

On this date, a Non-Compliance Conference was conducted with Licensee Marguerite Bruton. Present during the meeting were:

The following issues were discussed and citations are issued on the attached 809-D per Title 22 Regulations.

False Claims – Based on records reviewed and interviews conducted, Licensee Marguerite Bruton submitted an Administrator Certificate that was not issued by the Department. The Effective Date of: 5/30/2023 and Expiration Date of: 5/29/2025 were altered. The licensee last held a valid Administrator Certificate that has since expired on 05/29/2023.

Administrator - Qualifications and Duties – Based on records reviewed and interviews conducted, the facility has not had an Administrator in the facility since 2023. The licensee does not have a valid Administrator certificate and has not appointed an Administrator.

Exit Interview was conducted and Appeals Rights were provided.
