On 12/16/25, Licensing Program Analyst(LPA) M. Yang arrived unannounced to conduct case management visit for the purpose of checking on the health and safety of the residents in care. LPA met with Administrator Pamela Bradley.
Administrator informed LPA that excluded staff was hired on 06/16/22 and was terminated 10/05/22. Excluded staff last day working at the facility was on 09/28/22.
No deficiency was observed. A copy of this report was provided to Administrator. An exit interview conducted.
