On 05/29/24, Licensing Program Analyst (LPA) L. Salazar arrived at the facility unannounced to conduct an annual continuation to conduct record reviews. LPA was greeted by ED, stated the purpose of the visit and was allowed entry into the facility.
LPA reviewed a sample of records for residents and staff. Resident records were found to be complete and contain the required documentation. Staff files were observed to be complete with updated training records.
