Facility Evaluation Report
On 2/24/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA met with Administrator Breanne Perez to conduct facility inspection.
Currently, there are two (2) residents in placement, both residents were at day program at time of inspection. Residents attend day program Monday through Friday, 9:00 AM to 2:30 PM. Facility observed to be clean, odor free, and a comfortable temperature. Facility tour began in resident bedrooms. Bedrooms observed to be fully furnished and required accommodations. Resident bathroom toured, bathroom observed to have grab bars and skid resistant surfaces. Fixtures observed operational and water temperature measured at 112 degrees F. Kitchen toured, LPA observed facility to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Facility has knives locked in hallway closet. All common areas of the facility observed to have adequate seating for all residents in care. Medications observed to be locked and secured in medication cart, medications observed to have original labels and to be administered as prescribed.
Smoke detectors and carbon monoxide detectors present and observed operational during today's inspection. Fire extinguisher present with a date of service 10/30/2025. Last fire drill conducted on 2/01/26 and last disaster drill conducted on 1/01/2026 according to facility records. First Aid kit present and kept in hallway closet.
Outside of facility tour, all exits open free of obstruction. Facility has large covered patio area with seating for all residents. No hazards observed.
No deficiencies cited during inspection. Exit interview conducted and a copy of report provided for facility records.
