The inspector’s account

On 2/04/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit. LPA arrived, introduced self, and allowed entrance to facility. LPA met with House Manager, Breanne Perez and stated purpose of visit. Regional Supervisor, Daniel Esparza contacted by telephone and arrived a short time later to conduct visit with LPA.

Department received two separate incident reports for dates of 1/14/25 and 2/03/2025. On 1/14/2025, it was reported by a staff person that Staff 1 (S1) was observed to be yelling at Resident 1 (R1), an internal investigation was conducted. During investigation video footage was reviewed but were unable to observe any incidents, and footage contains no audio. Staff 1 denied allegation of yelling at resident, and R1 is unable to be interviewed due to limited communication. Staff 1 was moved to an alternate work site.

On 2/03/26, a second incident report was received of Staff 2 (S2) making inappropriate comments to Resident 2 (R2). LPA interviewed R2, who denied comments being made to them. Staff 2 was moved to an alternate work site.

Prior to LPA, facility conducted training for staff on 1/21/2026 regarding Mandated Reporting-Elder and Dependent Adult Abuse and Neglect, on 1/23/2026 regarding Mandated Reporting for Abuse and on 1/26/2026 regarding 1:1 supervision, client's rights, slumber policy, support staff duties, zero tolerance, professionalism, and Respect and dignity.

No deficiencies cited during Case Management visit.

Exit interview conducted and a copy of report provided for facility records.
