Facility Evaluation Report

On 3/26/26 at 3:00pm Licensing Program Analyst (LPA) J. Leffall conducted a case management regarding an Incident report received by the Department. LPA met with Program Director (PD) Lereese Delouth stated purpose of visit.

The department conducted interviews and reviewed records. Based on internal findings the staff attempted to speak over client during a special Interdisciplinary Team Meeting on 2/6/2026. Per report the staff has received disciplinary action, where S1 will be separated from company, S2 and S3 will receive corrective actions that include retraining and re-education on Personal Rights and the Behavior Support Plan.

The following deficiencies is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6.
