Facility Evaluation Report

On 2/24/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up regarding details about an incident report received to CCLD on 2/23/26. LPA met with Program Director (PD) Nikki Walker and stated the purpose of the visit. LPA interviewed staff and residents regarding Personal Rights for (R1). The facility is currently conducting an internal investigation. The facility will disclose the results of the findings of the internal investigation to CCLD at a later date.

At this time, the case management Needs Further investigation.
