Facility Evaluation Report
On 10/21/25, Licensing Program Analysts (LPAs) L. Salazar and S. Doucette arrived at the facility for a complaint investigation. During the course of the investigation, LPA's observed additional deficiencies. LPA's met with Administrator Edward Romero.
LPAs conducted records review and observed facility did not ensure that a written intake assessment was prepare d by a licensed mental health professional prior to acceptance of Resident R1 in 2023. LPAs did not observe a Functional Capabilities assessment for R1 that includes the propensity for behaviors. R1 has documented care needs that are in conflict with the needs of other clients in care. The program of services offered do not include psychiatric care.
Per California Code of Regulations, Title 22, Division 6, Chapter 6, deficiencies are being cited on the attached 809-D. If not corrected, the violations will have a potential risk to the health, safety, or personal rights of clients in care.
Exit interview conducted with Administrator and plans of correction were developed and reviewed by LPAs. A copy of this report and appeal rights were provided at the time of visit.
