The inspector’s account

On 11/18/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. LPA introduce self, stated the purpose of the visit and met caregiver Gurinder Kaur. Licensee (L1) Tina Malhi and Administrator (A1) Sharnpreet Malhi was called and arrived shortly. LPA toured facility with L1 and A1. Five clients were present during inspection.

The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside or outside. An adequate supply of perishable and non-perishable food was observed. The temperature is maintained for refrigerator at 37 degrees F and freezer at 0 degrees F. Fire extinguisher was observed with a service date of: 05/20/25. Fire drills last completed: 10/11/25. Medications observed locked under kitchen counter. MARs were reviewed and medications were checked. Sharps observed locked in kitchen drawer. Cleaning supplies and chemicals locked under kitchen sink and garage shelf. All bedrooms were observed to have the required furnishings and adequate lighting. Extra linens observed. The bathrooms were toured. Bathrooms were observed operational during inspection. Hot water temperature was tested at 119.3 degrees F in bathroom 1 and at 114.9 degree F and 119 in master bathroom. Master bathroom window screen was observed in disrepair. Outside of the facility toured. The side gate observed self-closing and free of debris. Adequate outdoor seating is observed to be available for clients. Carbon monoxide and smoke detectors were tested and observed to be operational. Half of clients' and sample of staff files were reviewed to have all the required documents.

A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6.

Exit Interview conducted. The following documents are requested and submitted to Fresno CCL by: 11/24/25. Forms requested: Lic 308, Lic 500, Lic 610D, Lic 9020, and control of property. A copy of this report and appeal was provided to Administrator, whose signature on this form confirms receipt of this report.
