Facility Evaluation Report
On June 6, 2026 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management regarding an incident that was reported on May 25, 2026 regarding R1 and an incident that occurred.
LPA spoke with Administrator Shena Hodges. Shena stated R1 had made verbal threats but there was no actual knife involved. Shena explained this is part of R1’s behaviors they display at times, and no body was harmed. The rest of the report indicated the threats were verbal.
No deficiencies observed and no citations issues during today's visit.
Exit interview was conducted and a copy of this report LIC809 was provided to Administrator Shena Hodges.
